Off-the-shelf cancer vaccines target antigens shared across patients, such as mutant KRAS or HER2 peptides, so they are made in advance rather than per person. Sipuleucel-T is still the only approved therapeutic cancer vaccine in the US; tolerance to self-antigens and weak past results hold them back.

Rindopepimut is a peptide vaccine against EGFRvIII, a mutant protein found only on some glioblastomas. After a phase 2 that beat historical controls, the 745-patient double-blind phase 3 ACT IV found no benefit in 2016, and tumours in both arms had lost EGFRvIII at recurrence, a lesson in antigen escape.
Sipuleucel-T was the first therapeutic cancer vaccine approved (2010): it lengthened survival in metastatic prostate cancer by about four months without shrinking tumours or lowering PSA.
