Glioblastoma disrupts the barrier heterogeneously (contrast enhancement) but infiltrating cells sit behind intact barrier. Strategies: lipophilic/small brain-penetrant drugs (temozolomide, lomustine, dordaviprone, vorasidenib), locoregional delivery (CAR-T, convection-enhanced), focused-ultrasound opening, LITT, and intra-arterial delivery.
