LUMINA used tumour biology, not just age, to pick women who can skip breast radiotherapy: with a luminal A profile and endocrine therapy, local recurrence at five years was about two percent.
LUMINA was a single-arm prospective study of 500 women aged 55 or older with grade 1 or 2 T1N0 luminal A breast cancer (oestrogen and progesterone receptor-positive, HER2-negative, Ki67 of 13.25 percent or less) who had breast-conserving surgery and endocrine therapy and no radiotherapy. Five-year local recurrence was 2.3 percent, 90 percent confidence interval 1.3 to 3.8 (Whelan and colleagues, New England Journal of Medicine 2023;389:612 to 619), supporting biomarker-based omission.
