INSIGHT, trial NCT05734105 sponsored by Deciphera and Ono, is a blood-test-selected trial testing whether matching the drug to the specific resistance mutation beats the standard second-line drug in gastrointestinal stromal tumour, comparing ripretinib with sunitinib in patients whose circulating tumour DNA shows KIT exon 11 plus exon 17 or 18 mutations. It builds on an exploratory finding in INTRIGUE that this genotype responded far better to ripretinib, plans to enrol 54 patients, lists primary completion for 2027 and has published no primary readout. OnCo links it to sarcomas and GIST, liquid biopsy, KIT as a target, ripretinib and the pairing of ctDNA KIT genotyping with kinase inhibitor selection. It is the first ctDNA-genotype-directed randomised trial in GIST, and whether such a small trial can be decisive is the open question.
