The test for leftover cancer cells found none, down to the sensitivity of the assay (often one cell in 100,000 or a million). It is the deepest remission we can measure and is becoming a treatment goal and trial endpoint in blood cancers.
In CLL, undetectable MRD at the end of fixed-duration venetoclax therapy predicts years off treatment and defines when to stop; in myeloma, MRD negativity at 10^-5 or 10^-6 (by next-generation flow or sequencing) is accepted by the FDA as an early endpoint for accelerated approval and guides de-escalation trials; in ALL, converting MRD-positive to negative with blinatumomab (BLAST) was the basis of its approval; in AML, NPM1 or flow MRD decides who needs transplant. MRD-positive after therapy predicts relapse and triggers intensification. The depth (10^-4 versus 10^-6) matters and assays are not interchangeable.
