Switching off testosterone, the hormone that feeds prostate cancer, with injections (or tablets) that stop the testicles making it, or by removing them. It has been the backbone of advanced prostate cancer treatment since 1941.
GnRH agonists (leuprolide, goserelin) and antagonists (degarelix, oral relugolix) lower testosterone to castrate levels; bilateral orchiectomy does the same permanently. ADT is combined with radiotherapy for intermediate- and high-risk localised disease (4-6 months to 2-3 years), and with ARPIs and sometimes docetaxel in metastatic hormone-sensitive disease. Side effects are hot flushes, fatigue, loss of muscle and bone, weight gain, sexual dysfunction and metabolic and cardiovascular risk, hence interest in intermittent schedules and in whether metastasis-directed therapy can postpone it. Endocrine therapy in breast cancer is the analogous concept for oestrogen.

The answer to a question three individual trials could not answer on their own, and the reason docetaxel with androgen deprivation became standard for metastatic hormone-sensitive disease. It also stopped a widely used bone drug being credited with a survival effect it does not have.
A negative trial that was later overturned, and a standing argument for pooling individual trials rather than acting on the first one to report. It is also the reason the distinction between high-volume and low-volume metastatic disease is written into guidelines.
The reason intermittent androgen deprivation is offered as a choice in metastatic disease rather than recommended, and a case study in what an inconclusive non-inferiority trial should say. For a man weighing the side effects, the honest statement is that the quality-of-life gain is real but brief and the survival question is open.

The first evidence that resistance to hormone therapy in prostate cancer is an adaptation of the target rather than an escape from it, which is why the field kept building better androgen receptor drugs instead of abandoning the pathway.

More than eighty years later, every man who starts androgen deprivation is having this experiment repeated on him, and the disease is still defined by whether it has stopped responding to it.
