Leads medical oncology at Tata Memorial and its programme of pragmatic trials that make treatments affordable.
Kumar Prabhash is Professor and Head of the Department of Medical Oncology at Tata Memorial Hospital, part of the Tata Memorial Centre in Mumbai. He leads the department's programme of pragmatic trials that make treatments affordable, including trials of oral metronomic chemotherapy in head and neck cancer, low-cost EGFR tyrosine kinase inhibitor strategies in lung cancer and low-dose immunotherapy studies. His publications include a trial comparing gefitinib alone with gefitinib plus pemetrexed and carboplatin chemotherapy in EGFR-mutated lung cancer. His interests are head and neck cancer, lung cancer, metronomic chemotherapy and pragmatic trials.

Patients newly diagnosed with EGFR-mutated advanced lung cancer now have a first-line option that improves survival over osimertinib, particularly if they have high-risk features. The trade-off is intravenous (now subcutaneous) infusions and considerably more skin, nail and clotting toxicity, so osimertinib alone remains reasonable for those who prioritise convenience and tolerability. Both this regimen and osimertinib plus chemotherapy (FLAURA2) are approved; there is no direct comparison.
For the majority of the world's head and neck cancer patients who cannot afford full-dose checkpoint inhibitors, a low dose added to oral metronomic chemotherapy is a tested alternative that improves survival. It also challenges the assumption that approved doses are the necessary doses: pharmacology had long suggested receptor saturation at far lower exposures.
