A blood-vessel-blocking antibody that shrinks glioblastoma on scans and reduces swelling, but has never been shown to help patients live longer.
Accelerated approval for recurrent glioblastoma (2009, full 2017) on radiographic response. AVAglio and RTOG 0825 (2014): PFS gain in newly diagnosed disease without OS benefit; EORTC 26101 at recurrence: PFS but no OS benefit with lomustine. Valuable for steroid-sparing control of oedema and radiation necrosis. A case study in pseudo-response and the limits of imaging endpoints in glioma.

Given by infusion or injection in a clinic or hospital outpatient department, so it is a Part B drug: Medicare pays 80% after the Part B deductible and the patient owes 20% coinsurance, uncapped in Original Medicare unless a Medigap policy applies. Same drug and coding as bevacizumab; the glioblastoma indication is on label.
Preferred biosimilar usually required; some plans apply additional review for recurrent glioblastoma.
20% Part B coinsurance on a high-cost infusion adds up quickly: Medigap Plan G or N, Medicare Advantage maximum out-of-pocket, Medicaid dual eligibility, or a charity fund are the usual buffers.
