DREAMM-7, trial NCT04246047 sponsored by GSK and published in the New England Journal of Medicine in 2024, is the trial that brought the withdrawn myeloma antibody-drug conjugate belantamab mafodotin back, beating a daratumumab-based standard on both progression and survival. It randomised 494 patients with relapsed myeloma after at least one line to belantamab or daratumumab, each with bortezomib and dexamethasone, met its primary progression-free survival endpoint with a large effect and showed a survival benefit in the 2024 update, while ocular events in about eighty percent were managed by dose holds; the drug was re-approved in the United States and European Union in 2025. OnCo links it to multiple myeloma, belantamab mafodotin, bortezomib and Sagar Lonial, and DREAMM-8 is concordant. Whether the eye toxicity is acceptable when bispecifics offer alternatives is the open question.
