The trial that set treatment for older patients with glioblastoma: three weeks of radiotherapy instead of six, with temozolomide added, extended survival without worsening quality of life.
CCTG CE.6 with EORTC 26062-22061 and TROG 08.02, trial NCT00482677, randomised 562 patients aged 65 or older (median 73) with newly diagnosed glioblastoma to short-course radiotherapy (40 Gy in 15 fractions) with or without concurrent and adjuvant temozolomide. Median overall survival was 9.3 versus 7.6 months (HR 0.67, 95% CI 0.56-0.80) and median progression-free survival 5.3 versus 3.9 months; with a methylated MGMT promoter, 13.5 versus 7.7 months (HR 0.53), and with an unmethylated promoter 10.0 versus 7.9 months (HR 0.75, 95% CI 0.56-1.01). Quality of life was similar in the two arms. Published by Perry and colleagues in NEJM in 2017. Two earlier trials had established the alternatives for older or frail patients: the Nordic trial (Malmström 2012; temozolomide alone or hypofractionated radiotherapy at least as good as standard six-week radiotherapy over age 60, and better over 70) and NOA-08 (Wick 2012; temozolomide alone non-inferior to radiotherapy alone, with MGMT methylation predicting who benefits from temozolomide).
