Starting immunotherapy right after chemotherapy, rather than waiting for relapse, lengthened survival by about seven months.
JAVELIN Bladder 100, trial NCT02603432 sponsored by Merck KGaA and Pfizer and reported in 2020, showed that starting avelumab immunotherapy immediately after platinum chemotherapy, rather than waiting for relapse, lengthened survival in advanced urothelial cancer. It randomised 700 patients without progression after four to six cycles of platinum chemotherapy to avelumab maintenance with best supportive care or supportive care alone and met its primary overall survival endpoint, making avelumab maintenance the standard after platinum for four years. OnCo links it to bladder cancer, avelumab and Thomas Powles. It was superseded rather than contradicted when EV-302 displaced platinum from first-line treatment for most patients, and it remains relevant where enfortumab vedotin plus pembrolizumab is unavailable or contraindicated.
