IDeate-Lung02, trial NCT06203210 sponsored by Daiichi Sankyo and Merck, is the pivotal phase 3 for ifinatamab deruxtecan, the B7-H3 antibody-drug conjugate that produced some of the highest response rates ever seen in relapsed small-cell lung cancer. It compares the drug with topotecan, amrubicin or lurbinectedin after one prior line, with objective response rate and overall survival as primary endpoints, following IDeate-Lung01, in which more than half of patients responded at the 12 mg/kg dose. OnCo links it to small-cell lung cancer, B7-H3 as a target, ifinatamab deruxtecan, topotecan and lurbinectedin. It is recruiting with no results, is positioned to compete with tarlatamab in the second line, and how the two should be sequenced is the open question.
