GI oncologist behind trials of KRAS, HER2, and FGFR-directed therapy in colorectal and bile duct cancer.
Tanios Bekaii-Saab is Professor of Medicine and Leader of the GI Cancer Program at Mayo Clinic Arizona, part of the Mayo Clinic Comprehensive Cancer Center. A medical oncologist specialising in colorectal and biliary cancer, he leads gastrointestinal cancer research in Arizona and has been an investigator on ANCHOR CRC, MOUNTAINEER and FGFR2 inhibitor trials in cholangiocarcinoma. His papers include the New England Journal of Medicine report of adagrasib with or without cetuximab in colorectal cancer with mutated KRAS G12C and the ReDOS phase 2 study of regorafenib dose-optimisation in refractory metastatic colorectal cancer. A frequent voice on colorectal cancer treatment sequencing, his work spans KRAS, HER2 and FGFR2 in colorectal, biliary tract and pancreatic cancer.
Adagrasib joins sotorasib as a later-line option for KRAS G12C pancreatic cancer in guidelines; both drugs are the template for the G12D and pan-RAS inhibitors now in pancreatic trials.
For fit patients with newly diagnosed metastatic pancreatic cancer, a FOLFIRINOX-type regimen is now proven to be better than gemcitabine plus nab-paclitaxel, settling a long-standing debate. The absolute gain is about two months of median survival, and the regimen is more toxic for the gut. Whether liposomal irinotecan adds anything over conventional irinotecan (standard FOLFIRINOX) has never been tested head-to-head.
