Project ECHO is a weekly video class where district doctors and nurses present real cases to a specialist team, learn by doing, and build a network. It worked for hepatitis C and could work for cancer.
Project ECHO's hub-and-spoke tele-mentoring model, in which specialists at a hub run regular case-based video sessions with community clinicians, has shown outcomes for hepatitis C treated in primary care equal to specialist clinics. Oncology ECHO programmes exist in India, Africa, and the US but are small. Scaling structured ECHO for symptom management, palliative care, and common-cancer protocols would multiply the reach of scarce specialists.
