Most chemotherapy is safe after the first trimester, surgery is safe throughout, and ending the pregnancy does not improve the mother's outcome.","summary":"Incidence is rising with later childbearing. Principles (INCIP, ESMO 2013/2019 guidelines): staging with ultrasound and non-contrast MRI, avoid radiotherapy to the abdomen and most targeted/immune agents; surgery any trimester; anthracycline- and taxane-based chemotherapy from 14 weeks with delivery ≥3 weeks after the last cycle and not before 37 weeks where possible; no chemotherapy in the first trimester (malformation risk). The INCIP cohort showed children exposed in utero have normal cognitive and cardiac development at 3 and 6 years (Amant, NEJM 2015), with prematurity, not chemotherapy, driving outcomes. Trastuzumab (oligohydramnios), endocrine therapy, immunotherapy, TKIs and methotrexate are contraindicated or avoided. Pregnancy-associated breast cancer within a few years postpartum has worse biology. Placental metastasis is rare (melanoma most common).","asOf":"2026-09-08","links":[{"label":"ESMO cancer in pregnancy guideline","url":"https://doi.org/10.1093/annonc/mdt199"},{"label":"INCIP (Amant, NEJM 2015)","url":"https://doi.org/10.1056/NEJMoa1508913"},{"label":"INCIP network","url":"https://www.cancerinpregnancy.org/"}],"tags":["gap-fill"],"related":["breast-hr-positive","cervical","hodgkin-lymphoma","melanoma","gestational-trophoblastic","fertility-preservation"],"cancers":[],"sections":[],"technologies":[],"targets":[],"drugs":[],"companies":[],"institutions":[],"pathways":[],"terms":["aya-oncology"],"trials":[],"people":[],"bottlenecks":[],"keyPapers":["paper-amant-n-engl-j-med","paper-peccatori-ann-oncol"],"journals":[],"dependsOn":[],"notes":[],"category":"Clinical","wikipediaChecked":"2026-09-22"},"route":"/terms/cancer-in-pregnancy/","neighbours":{"cancer":[{"id":"aya-cancers","kind":"cancer","name":"Adolescent and young adult cancers (ages 15 to 39)","route":"/cancers/aya-cancers/"},{"id":"cervical","kind":"cancer","name":"Cervical cancer","route":"/cancers/cervical/"},{"id":"gestational-trophoblastic","kind":"cancer","name":"Gestational trophoblastic neoplasia","route":"/cancers/gestational-trophoblastic/"},{"id":"hodgkin-lymphoma","kind":"cancer","name":"Hodgkin lymphoma","route":"/cancers/hodgkin-lymphoma/"},{"id":"breast-hr-positive","kind":"cancer","name":"HR-positive / HER2-negative breast cancer","route":"/cancers/breast-hr-positive/"},{"id":"melanoma","kind":"cancer","name":"Melanoma","route":"/cancers/melanoma/"}],"technology":[{"id":"fertility-preservation","kind":"technology","name":"Oncofertility and fertility preservation","route":"/technologies/fertility-preservation/"}],"term":[{"id":"aya-oncology","kind":"term","name":"Adolescent and young adult (AYA) oncology","route":"/terms/aya-oncology/"}],"paper":[{"id":"paper-peccatori-ann-oncol","kind":"paper","name":"Cancer, pregnancy and fertility: ESMO Clinical Practice Guidelines for diagnosis, treatment and follow-up","route":"/key-papers/paper-peccatori-ann-oncol/"},{"id":"paper-amant-n-engl-j-med","kind":"paper","name":"Pediatric Outcome after Maternal Cancer Diagnosed during Pregnancy","route":"/key-papers/paper-amant-n-engl-j-med/"}]}}
