## TL;DR

A chemotherapy-free bispecific plus ADC doublet beat salvage chemotherapy in relapsed large B-cell lymphoma.

## Summary

SUNMO, trial NCT05171647 sponsored by Roche and reported in 2025, showed that a chemotherapy-free doublet of the CD20 bispecific mosunetuzumab and the CD79b antibody-drug conjugate polatuzumab vedotin beats rituximab-gemcitabine-oxaliplatin salvage chemotherapy in relapsed or refractory large B-cell lymphoma in transplant-ineligible patients. It randomised 208 patients and met its dual primary endpoints of progression-free survival and overall response, with a much higher complete response rate, in a fixed-duration outpatient regimen. OnCo links it to lymphoma, mosunetuzumab, polatuzumab vedotin and the pairing of a CD20 bispecific with a CD79b ADC. Overall survival is not yet mature, and whether the doublet is an alternative for patients not fit for CAR-T or a competitor to it is the open question.
