RefleXion Medical, based in Hayward, California, makes the first PET-guided radiotherapy system, which uses the tumour's own positron emissions to steer the treatment beam. Its SCINTIX biology-guided radiotherapy received FDA authorisation in 2023 for lung and bone metastases. OnCo links it to MR-guided adaptive radiotherapy and PET as technologies, to the molecular imaging roadmap from FDG through PSMA and FAP to immune PET, to the bottleneck that surgery and radiotherapy cure most patients but get least funding, and to the idea of coverage-with-evidence registries for adaptive radiotherapy.
