## Summary

The CHALLENGE trial showed that a supervised exercise programme improves outcomes in colon cancer, and this idea extends the same rigour to breast, prostate and lung cancer, prescribing structured exercise like a drug in all curative-intent care. A three-year programme after curative treatment is expected to improve disease-free survival through insulin and IGF reduction, lower inflammation and immune surveillance. Level-1 evidence now exists, so implementation is the barrier: reimbursement, workforce and adherence. The test is CHALLENGE-design trials in TNBC and prostate cancer with health-economic analysis and digital delivery arms.
